The phased,
multivendor PACS implementation at Massachusetts General
Hospital features a modular digital infrastructure that
is Internet enabled and fully expandable.

Thrall, MD Since the discovery of x-rays in 1895,
medical imaging has become a key factor in health care
delivery. For more than 100 years, radiographic film has
been the primary tool for capturing and interpreting
disease processes. As new techniques and modalities over
the past several decades have increasingly embraced
digital imaging, including ultrasound, digital
angiography, computed tomography, and magnetic
resonance, a natural evolution to digital-based image
management has become compelling. The concept of the
picture archiving and communications system (PACS)
encompasses this logical extension, and as such,
multiple major PACS installations are now under way. As
leaders across the country begin to embark on the task
of implementing the filmless environment, many look to
current installation sites for a better understanding of
how to manage the process. In the early months of
1995, the Massachusetts General Hospital (MGH), Boston,
embarked on a program to eliminate film in an attempt to
improve radiology department operations and reduce
departmental costs. Prior to the generation of our Phase
One request for proposal, we identified essential core
components for our implementation. We systematically
analyzed our current workflow, the proposed workflow,
and the technologic basis of implementation. We
identified key components including the primary archive,
the network, acquisition devices with digital imaging
and communications in medicine (DICOM) modifications,
workstations, intranet distribution, and voice
recognition. In this article, we will discuss our
experiences with a phased installation and discuss
technology we have employed to achieve our goals.
